Health Assessment
Complete a short online questionnaire each year by the close of Annual Enrollment — November 30 — and save $800 on your health plan premiums next year.
That's $30.77 back in every paycheck if you work a 12-month schedule or $36.36 per paycheck if you work a 10-month schedule. Completing the Health Assessment also unlocks MotivateMe, where you can earn up to $100 in gift cards each year.
What is the Health Assessment?
The Cigna Health Assessment is a brief, confidential questionnaire about your health, stress, work satisfaction and lifestyle. It takes just a few minutes. When you finish, you'll receive a personal report identifying any health risk factors — something you can review on your own or share with your health care provider.
How to Take It
- Log into myCigna (or register if it's your first time).
- Select Health Assessment from the Wellness tab.
- Answer all questions and submit.
Your Privacy is Protected
Your individual answers are protected under federal health privacy law and are never shared with MNPS. The company receives only aggregated, anonymous data to help shape wellness programming — nothing that identifies you personally.
If your results suggest follow-up, you may hear from an onsite health coach or a Cigna health advocate. Neither will ever share your personal information with MNPS. You can review MNPS's full privacy practices on Oracle Cloud.
A Few Important Details
- Your spouse doesn't need to complete the assessment for you to qualify for the premium discount.
- Biometrics are not required — but if you have recent bloodwork, you're welcome to use those numbers when you take the assessment.
- Miss November 30 deadline? You can still take the assessment anytime during the year, but your discount will be prorated based on when you complete it. It takes up to two pay periods (about 30 days) for your discount to take effect. The sooner you complete it, the more you save.
