Your vision plan, administered by EyeMed, covers annual eye exams, eyewear and contact lenses — with no deductible. You get the most value when you use providers in the EyeMed Insight network.

What You Pay (In-Network)
See the chart on this page for your cost breakdown. Most covered services cost you just a $10 copay. Frames are covered up to $120 retail (then 20% off the balance), and contacts are covered up to $120.
Important: Each year, you can use your eyewear benefit for either frames and lenses or contact lenses — not both.
If you go out-of-network, you'll pay the provider upfront and submit a claim for partial reimbursement. Out-of-network allowances are lower than in-network benefits.

- To find an in-network provider, visit the EyeMed website
- Select Find an Eye Doctor menu.
- Select the Insight network from the dropdown menu.
