Life Insurance

Life insurance and accidental death & dismemberment (AD&D) insurance provides financial security for the people who depend on you, and is one part of your protection benefits.

Disability insurance is the other part, protecting your income if illness, injury or pregnancy keeps you from working.

Life and AD&D insurance is provided through Dearborn National.

What is AD&D? This coverage pays an additional benefit on top of your life insurance if you die in an accident — doubling your payout. It also pays a partial benefit if you survive an accident but lose the use of a body part, such as an eye or a limb.

Filing a Claim

To file a life insurance claim, call Dearborn National at 1-800-348-4512. You must present a certified copy of the death certificate to file a claim.

MNPS-Provided Coverage

MNPS automatically provides you with:

  • A $500 afterlife benefit
  • $50,000 in basic life and AD&D insurance at no cost to you

You receive this coverage even if you don't elect health plan coverage.

Supplemental Life Employee Coverage

You may increase your financial protection by electing supplemental life and AD&D coverage.

You can purchase supplemental life and AD&D coverage in increments of $10,000, up to $500,000 — on top of your MNPS-provided basic coverage.

Premiums are based on your age and are paid on an after-tax basis. See the Plan Rates page for your cost.

Dependent Life Coverage

If your dependents are enrolled in the Certificated Employee Health Plan, they're automatically enrolled in the following basic life coverage at rates shown on the Plan Rates page:

  • Spouse or domestic partner: $25,000
  • Child over 6 months: $10,000
  • Child (14 days to 6 months): $5,000
  • Child (0-13 days): $2,500

If your dependents are not enrolled in the health plan, you can elect this coverage at the same rates on an after-tax basis.

Proof of Good Health

In certain cases, Dearborn will require you to submit proof of good health — also called evidence of insurability, or EOI — before coverage takes effect. This applies if:

  • You decline supplemental or spouse/domestic partner life when first eligible but wish to elect it later
  • You want to increase your supplemental employee life coverage during Annual Enrollment
  • Your total life and AD&D coverage exceeds the guaranteed issue amount of $300,000

Additional Benefits

Your Dearborn National life insurance policy includes these additional benefits:

  • Seatbelt benefit: $10,000 if you suffer a loss while wearing a seatbelt; an additional $5,000 if an airbag deployed
  • Terminal illness benefit: If you or a covered dependent under age 60 is diagnosed with a terminal illness and given less than 12 months to live, you can request up to 100% of your life insurance benefit early.
  • Travel benefit: If death occurs more than 75 miles from home, up to $5,000 is provided for transportation or cremation preparation.
  • Grief, legal and financial counseling for beneficiaries: provided at no charge — call 1-800-778-2281
  • Disability waiver of premium: if you become totally disabled before age 60 and your disability lasts at least six months, your premiums may be waived while you remain disabled, up to age 65.
  • Child care benefits for surviving children under 12
  • A coma benefit for individuals who have been in a coma for at least 31 days
  • Dependent child education benefit to help current students complete post-secondary education
  • Spouse education benefit to help a surviving spouse pay for higher education

Benefit Reductions

Basic and supplemental employee and spouse life benefits:

  • Are reduced to 65% of the original coverage amount at age 65
  • Are reduced to 50% of the original amount at age 70 
  • End at employee's retirement
